What should I bring on my first visit?
Please arrive 10-15 minutes prior to your first visit in order to complete the necessary paperwork.Â If you prefer, you can download the forms on this site in advance to complete and bring with you. You will need to bring your insurance card, prescription from your MD if applicable, any radiographic test results (Radiology reports for X-rays, CT scans, MRIs, etc.) and a check or credit card for any payments. You should bring comfortable clothes including shorts, athletic shoes and a tee shirt, tank top, and/or sports bra.
Do I need a prescription for Physical Therapy?
Due to the Direct Access law in New York State you are entitled to see a physical therapist for up to 10 visits or for 30 days from your initial assessment (whatever comes first). To qualify, your referring physician must practice in New York state or be within 25 miles of NYC. We recommend getting a referral from your doctor for physical therapy services, as some insurance companies will not reimburse for services performed without a physicianâ€™s prescription. If you donâ€™t have a doctor, we will be happy to recommend one.
What Insurances do you accept?
We are an out-of-network provider with all insurance companies, and a non-participating provider with Medicare (we will electronically bill on your behalf and you will receive a check directly back from Medicare). We will check your insurance benefits and email you prior to your appointment to determine a billing arrangement based on your coverage. We will bill the insurance company directly on your behalf. You may be responsible for a co-payment or a percentage of the bill at the time of service.Â If you donâ€™t have out-of-network insurance benefits we do extend a discounted cash-pay rate due at the time services are rendered. If your insurance benefits run out during the course of physical therapy we will also extend a discounted cash-pay rate. We will happily answer any further questions you may have regarding reimbursements.
Late Cancellations/No Show Policy:
We understand that unanticipated events occasionally happen. In order to be effective and fair to ALL of our clients, and out of consideration for our therapists and specialistsâ€™ time, we have adopted the following policies:
- If you miss or cancel your appointments without giving us 24 hours notice, we reserve the right to charge a late cancellation fee, which is not reimbursed by insurance.
- If your appointment is on a Monday, please notify our front desk by noon on the Friday before.
- Please give 24 hours notice for cancellations.
Appointment times have been reserved exclusively for you. If you arrive late, you will be responsible for the full bill and your session may have to be shortened in order to accommodate the patients whose appointments directly follow your session.
